When your shift ends, the sheet grinds up like it went through a shredder and it's gone. Patient information never leaves this phone. No server, no account, nothing to sign into.

BrainSheet is a nurse report sheet (brain sheet) for one nurse and one shift. Open it and today's patient grid is already laid out.

Start of shift
Pick your unit (Med-Surg, ICU or Tele), day or night, and how many patients you have. The grid lays itself out.

During the shift
Room, initials, code status, allergies, things to watch for. Meds and treatments sit on a timeline, and "Right now" keeps what's due in the next hour at the top. One tap checks it off. Notes and lab values go in with a glove on, in the hallway, in ten seconds.

Handoff
One screen per patient in SBAR order, swipe for the next. It is the cheat sheet you glance at while you talk, not a document. So there is no export, no PDF, no print and no sharing.

End of shift
Tap Shred and the sheet tears into strips and falls away. The data is erased right then. Forget? After your shift ends and the delay you set has passed, it shreds itself before you even open the app.

Nothing to leak in the first place
There is no field for a full name, date of birth or MRN. A room number and initials are enough. Stored outside iCloud backup, hidden in the app switcher, and you can lock it with your fingerprint or face.

Works with no signal, no Wi-Fi and no login.

A personal note-taking tool. Not a medical device, not an EMR, not a charting tool, and it does not help with diagnosis or medication decisions. Follow your facility's policies first.
