HomeTeams is a family health and caregiving app that brings family, friends, neighbors and caregivers into one shared space — a shared family calendar, to-do lists, medication reminders and secure medical records, with an AI mentor and guide. HomeTeams is not a medical device and does not diagnose, treat or prevent any condition.

Whether you're managing a health condition, caring for an aging parent, or supporting a child, HomeTeams helps you make a clear plan and share it with the people who support you. YOUR HEALTH RECORDS, PULLED IN FOR YOU

Verify your identity once and HomeTeams gathers your medical records for you — hospital stays, the urgent care visit you forgot about, lab results, clinical notes and medications, from the places you've been treated. New records keep arriving as care happens, so the picture stays current. Ask Coach to explain any of it in plain English.

Add your own health notes and documents, and choose which teammates see what. Keep insurance cards and forms in the Resource library. Reach a nurse from the Health tab when you need a real person.

TEAM HOME

No more scattered texts or unclear responsibilities. Team Home is a shared feed of what happened, what is next and who has it covered. Team Captains control what each teammate can see, and you can switch teams if you support more than one household. Team, group and direct messaging keeps the conversation in one place.

GUIDED SETUP IN MINUTES

Answer four questions out loud and Coach assembles your team, or type them yourself. Name the team, add the first appointment, send invites.

SHARED TASKS AND FAMILY CALENDAR

Create tasks — we call them actions — for errands, doctor appointments, rides, meals, medication and quality time. Choose who each task is for and who does it, set a date or leave it open, and sort with color-coded categories. Filter by category, status or day. When a task looks like a medication, HomeTeams sets it up with dose, strength and reminder times.

COACH, YOUR MENTOR AND GUIDE

Coach is the AI at the heart of HomeTeams. It answers the question in front of you at 11pm, in plain words — Coach knows the plan, the medications, and who to pull in. Ask by voice or text, and a daily digest summarizes your team's activity.

HOMETEAMS WALLET

Link a card you already use and cash back accumulates at participating merchants, tagged to your team's goal. Relatives can contribute directly to shared Team Funds. Real cash, not points, not coupons.

BRAE PROGRAMS

Structured mind, body and soul health plans: living with a chronic condition, building the mental toughness caregiving demands, or working through forgiveness. Short daily wellness practices and gentle accountability.

LIVING LEGACY

Capture a life story in the person's own voice, one gentle question at a time. Old photos are restored and kept in a private family vault.

HEALTH CONNECT (OPTIONAL)

Connect Health Connect to share steps, sleep and fitness activity with your HomeTeam. This data is used only for care coordination inside Health Insights. It is never used for advertising and never sold.
