This app is for Medicare-eligible patients who have signed up and established an account as recommended by their healthcare provider. This app functions as a data gateway, transmitting patient vitals data from integrated vitals monitoring devices such as a blood pressure monitor. The app provides patients with the ability to view their vitals data in detail as well graphically. The patients receive messages and prompts encouraging them to take their readings regularly. This app is an extension of a comprehensive remote patient monitoring (RPM) program that requires monthly calls by trained and credentialed Care Coordinators to the patient. The program is administered on behalf of the patient’s healthcare provider.
