About ECG Rhythms: Practice Strips

Most ECG apps ship a fixed library of tracings. Work through it twice and you stop reading the strip: you start recognising the picture. Then the real thing looks nothing like the picture. This app generates every strip from scratch. Pick atrial fibrillation and you get a different atrial fibrillation every time, with its own rate, its own irregularity, its own baseline. You cannot memorise your way through it, which is the point. WHAT YOU GET Thirty-one rhythms, drawn on a real 25 mm/s, 10 mm/mV grid with a calibration pulse. Sinus rhythms including arrhythmia and pause. Atrial fibrillation and flutter. Supraventricular and junctional rhythms. All four AV blocks. PACs and PVCs, the R-on-T phenomenon, ventricular tachycardia, torsades de pointes, idioventricular and agonal rhythms, ventricular fibrillation and asystole. Paced rhythms, atrial, ventricular and dual-chamber, plus failure to capture and failure to sense. CHOOSE HOW HARD IT IS Answer from a list while you are learning, then switch to open response and name the rhythm yourself with nothing to pick from. Shorthand works, so afib, vtach and Wenckebach all find it. Set the signal quality. Clean is the textbook tracing. Realistic adds the wander and muscle noise of a cooperative patient. Noisy is the strip nobody trains on and everybody eventually gets. SEE WHY, NOT JUST WHETHER Every answer comes with the criteria that define the rhythm, the measured rate, PR, QRS and QT off that exact tracing, and an explanation of why it is not the rhythm you confused it with. MONITOR MODE Recognising a rhythm as it sweeps across a monitor is a different skill from reading a still strip, and it is the one you need at the bedside. Almost nothing else trains it. TRACK WHAT IS STILL SHAKY The rhythms you miss come back more often. The progress page tells you which ones you have down, which ones keep catching you out, and which pairs you are mixing up. New: Measure It. Naming the rhythm is only half the exam. Measure It asks for the rate, the PR interval, or the QRS duration on the strip in front of you, and grades what you type against the strip's own numbers. Not every quantity exists on every rhythm; a strip with nothing fair to measure tells you that, which is itself the finding. Fixed: the rate-counting hint now says to turn the phone. The strip is six seconds, but portrait only shows about three, so counting complexes in portrait was giving half the real rate.
