About PHR Health Records
PHR Health Records is a private, on-device health record manager for iPad — for you and your whole family. Track medications, conditions, allergies, lab results, vitals, immunizations, procedures, treatments, insurance, care team, facilities, legal documents, and visits all in one place. No accounts. No cloud. No subscriptions. PRIVATE BY DESIGN Every patient's data lives in a sandboxed database on your iPad. PHR never uploads or syncs your health records. The only times data leaves your device are ones you initiate — and PHR tells you exactly what's going where before it does. IMPORT FROM YOUR PATIENT PORTAL Download a CCDA file from any patient portal and bring conditions, medications, allergies, lab results, immunizations, procedures, contacts, and patient info directly into PHR. Every importer shows you what will land before you commit, with per-batch undo. PULL LAB RESULTS FROM APPLE HEALTH Connect to Apple Health on your iPad and import them directly from your providers' systems — with smart matching that won't double-import data you already have from a CCDA. AI HEALTH CHECK Run a complete review of your health record on demand. PHR strips out names, dates of birth, addresses, phone numbers, emails, and record numbers, then sends a de-identified snapshot to your chosen AI service for a structured analysis you can take to your next visit. ASK AI WITH MEDICAL RESEARCH Turn on Healthcare Mode in the Claude setup and your questions get routed through Anthropic's PubMed research connector. Answers come back with clickable links to the actual peer-reviewed papers cited. YOUR CHOICE OF AI Claude, OpenAI, Google Gemini, xAI Grok, or Perplexity — bring your own API key, your own account. Each provider's data policy is shown before you tap Ask. SMARTER MEDICATIONS NIH RxTerms drug lookup, FDA drug-label info, drug-interaction checking across your active meds. Custom column layouts on the Medications list — choose which columns show, in what order, save as named views. REAL REPORTS Combined Health Summary, Lab Results Matrix, Travel Health Summary, Weekly Medication Checklist, Drug Interaction PDF — every report renders as paginated US Letter PDF you can email, AirDrop, or print. ATTACHMENTS Attach PDFs, photos, and other documents to Reports, Procedures, Legal Documents, Treatments, Insurance, and Visits. Stays on device. LAB RESULTS DONE RIGHT LOINC-standardized test names so the same lab from different providers shows up under the same name. Filter by panel, test, indicator, date range, ordering provider, or LOINC code. Multi-test graphs spot trends across years. FOR FAMILIES Manage up to 10 patients on one iPad. Owner controls who sees what; users see only their own data. Backup and restore to the location you choose. Transfer to a new iPad without losing a thing. PHR is built and supported by Evans & Associates PHR, LLC. Contact us at [email protected] — we read every message. PHR is for personal use — not a HIPAA-Covered Entity tool, not for use by providers or insurers. AI features are for information only and are not a substitute for professional medical advice, diagnosis, or treatment.
What’s new
Version 2.4.0 · Updated October 22, 2026
A fresh new look, and a smarter way to manage your medications. NEW LOOK Every record screen — Medications, Conditions, Allergies, Visits, Immunizations, Insurance, Care Team, and more — now groups its details into clear cards, with status badges you can read at a glance. Phone numbers and email addresses can be selected to call or write. Visits show a follow-up tag that turns red when you’re overdue. Insurance has a “Show at the counter” card with the numbers a pharmacy asks for. Immunizations can now hold attached documents, like a photo of your vaccination card. MEDICATIONS Discontinue a medication instead of deleting it — its history is kept, along with the reason it was stopped. Restart a stopped medication with one button. Set a different amount for each time of day, like 2 in the morning and 1 at night, and see your total per day. Put your medications in your own order, and star the main condition each one treats. AI Updated for the latest models from Google, Anthropic, OpenAI and xAI, with refreshed setup directions — including Google Gemini’s free tier. More accurate cost estimates for AI answers. ALSO Import a contact straight from your Contacts app. Inactive providers now appear in every provider list. New visit types: Skilled Nursing and Long Term Care. Faster Transfer Patient for large records, and many smaller fixes.
