Dedicated referral forms for physicians, as well as a form specifically for community partners like facility administrators/activity directors/discharge planners. We understand that professionals in different areas may notice unique signs in their patients, and so our referral process is built to ‘speak your language’. You can even upload facesheets and clinical photos, right from the app!
Email confirmations for any referral you make, as well as an in-app listing of all of the patients you have referred to our services.
