About PICUCalc: Pediatric ICU

PICUCalc is the most complete clinical reference for Pediatric Intensive Care: infusions, antibiotics with automatic renal dosing, 40+ validated scores, and emergency protocols — all in one app, fully offline.

PATIENT
Enter weight, age and height once: BMI, body surface area (Mosteller) and maintenance fluids (Holliday-Segar) are calculated instantly and feed the rest of the app automatically.

INFUSIONS
43 drugs with weight-based preparation and infusion rate: vasoactives, sedation, analgesia, neuromuscular blockade and more. Peripheral-line dilution for the 6 classic vasopressors. N-acetylcysteine with both protocols. Hypertonic saline at 3/6/9% from 20% NaCl.

ANTIBIOTICS
60+ antimicrobials with daily dose, per-dose amount, dose caps and AUTOMATIC RENAL DOSING across 4 renal-function bands (based on Nelson's Pediatric Antimicrobial Therapy). Indication-specific dosing, for example trimethoprim-sulfamethoxazole for pneumonia or prophylaxis (by body surface area).

SCORES (40+)
Sepsis: qPS4, pediatric sepsis phenotypes and PODIUM (10 systems, 43 criteria). Gastroenterology: PELD, PROVE-ALT, MELD/MELD-Na, King's, PUCAI, STRONGkids. Pulmonology: Westley, Wood-Downes-Ferres, PRAM, C-ACT, GINA, Light's criteria, asthma and bronchiolitis phenotypes. Rheumatology: SLEDAI-2K, PGA, BILAG. Neurology: GMFCS and PedNIHSS. Emergency: PECARN, ESI, JumpSTART. Renal: KDIGO. Plus sedation/analgesia, delirium and diagnostic criteria.

CALCULATORS
Blood gases (Henderson-Hasselbalch and Stewart), oxygen transport with cardiac index for ECMO, ROX, OI/PaFi/SaFi, GFR, renal angina index, sodium and total body water, dehydration, diabetic ketoacidosis, plasma exchange, bidirectional GIR, dengue hydration (PAHO/WHO), lactate.

EMERGENCY
Pediatric CPR with timer and voice alerts per PALS 2025, and status epilepticus with weight-based dosing.

Every calculation cites its source: ISPAD, PALS, AES, ESPGHAN/ESPEN, PAHO/WHO, Nelson's, Taketomo and the original paper behind each score.

WHY IT'S DIFFERENT
It integrates the full PICU workflow in one app: an antibiotic engine that recalculates the dose against real-time renal function, peripheral dilution when there is no central line, and cardiac index for ECMO. Built by a practicing pediatric intensivist for real bedside work.

Available in English, Spanish, Portuguese and French. Works completely offline, collects no personal data, no ads.

A clinical reference tool for support. It does not replace the treating physician's judgment. Always verify doses and parameters against your institutional protocol before administering.
