Healthfort mobile application is a personal health record management and communication platform. It allows the users to store and readily access their health related documents like prescriptions, investigations, their digitized summaries, etc. through their mobile phones.

Healthfort provides you the ability to track key vital parameters like weight, BP, temperature, and biochemical parameters like Blood Sugar, Cholesterol, Hemoglobin levels, etc. These trackers are very useful especially in the management of chronic conditions and during the post-operative follow-up.

It allows parents to track their child’s vaccination online and receive timely phone alerts. Parents can track the growth of their children through growth charts.

Healthfort App allows users to receive important reminders like – medication reminders, follow up investigation and doctor visit reminders. Users receive regular health messages related to their health status / condition.

Healthfort App connects the patients to their treating doctors for tele-consultation requests. Patients have the ability to send key documents like blood reports, vital parameter graphs and also provide their doctor access to their Personal Health Record so that s/he can provide a more informed consultation.
