My name Dr Ryizan Nizar MD MRCPUK and I am determined to make diabetes care more accessible and affordable to everyone.

Where you live or what you earn should not determine the Diabetes care you get!

This App offers people the opportunity to store and track their health data for a small yearly subscription. It will also graph all your data and show what your targets are as well as your current standing in relation to such targets.

My DiabetesConnect will do all the thinking for you.


The dashboard:- With this one screen you can see an overview of your diabetes control with colour-coded tiles that change colour to indicate which aspects need urgent action/above target/at target. The tiles will also notify when a certain test is overdue. Now you are in complete control.

PDF Reports:- Create and share PDF reports with your doctor in an instant. This will help your doctor make more informed decisions by analysing all your records over months/years.

Blood Glucose Screen:-This app has meal specific targets and you can instantly know if a certain blood glucose reading is high with the App's easy to understand colour coding. Readings will go red if it needs immediate attention, amber if high and green if in target.


BMI Screen- App will auto calculate your target weight once you enter your height and you will be able to easily see your progress in achieving these target.


HbA1c Screen:- Track your HbA1c over several years and you can easily track your progress, after all, Diabetes is all about trends.


Blood Pressure:- Now it's easy to track your blood pressure with My DiabetesConnect, know when it is too high and you can see trends over months/years.


Kidney Function (Blood test):- Track your kidney function overtime to make sure they are healthy. See the trends and know early if kidney function is deteriorating.


Kidney Function(Urine test):- Often this is the first sign of trouble that would indicate diabetes is starting to affect your kidney. Knowing early can help your doctor treat early and prevent/delay kidney failure.

Cholesterol:- Track your cholesterol over months/years. It's all about trends.

Eye and Feet Check:- Keep a track of your eye and feet screening. The App will remind you when you are due for your next eye appointment.

Medication:- Now you can keep track of all your medications with this one App. What's more, you can also store all your stopped medications so your doctor knows what you had been on before. that didn't work.

Diagnosis:- Keep track of all your diagnoses.
