Features
You can:

- View or email your ID card to your provider.
- Request a replacement ID card [Can we print?]
- View your plan benefits [One issue is that I can’t currently resize the PDF on my phone or have it go horizontal so I can’t actually read the benefits]
- Find out if you’ve reached your out-of-pocket maximum or deductible
- Review claims history
- Find out if an authorization has been approved
- Request a new Primary Care Provider (PCP)
- Access an online provider search

Security
You must always sign in with your User Name and Password to access the features in this app. Without that information, no one can get your personal data. You can sign in or sign up on the app or on the PHP member portal at prominencehealthplan.com.
