LINK is Kern Family Health Care’s (KFHC) member app for our Member Portal where KFHC members can get health plan information online. LINK provides self-service tools that can be used through your cell phone, tablet, or other mobile device!

Do you want to know if it is time for your annual health checkup? LINK-up and find out with the Member Portal Health Care Reminders. This feature lets you see if you are due for your annual health checkup and tells you other important health care reminders based on your health status.
Do you need to find a doctor? LINK-up and click on “Find A Provider”. By logging on and clicking on the “Find A Provider” menu option, you can look for a doctor who is a PCP or a Specialist, who is male or female, and who speaks a second language.
Do you need a ride to your appointment? LINK-up to find out how you can request this service that is free of charge to KFHC Members.
Did you have a baby? LINK-up to add your baby to your KFHC account! Newborns are covered under your KFHC benefits for the month of birth, plus the month following. This feature also lets you attach documents if needed.
Do you need a KFHC ID Card? LINK up to request a new ID card or view your ID card from your mobile device and don’t forget to let us know if you moved by changing your address in the “Demographic Changes” menu option.
Would you like to review your KFHC health care benefits? LINK-up to see your KFHC Member Handbook! All your benefits and restrictions are available at your fingertips.
Is there another person you would like to have access to your account? LINK-up to authorize that person to have access to your account. Remember, KFHC is required to keep your information private. You must let us know if you want to share your Protected Health Information with an authorized person.
Do you prefer to speak a language other than English? KFHC will provide you with translation services. LINK-up to find out about our interpreter and translation services.
Do you need to speak to an advice nurse? KFHC offers nurse advice to KFHC Members. If you need to talk to a health care professional, or if you want to listen to recorded messages through our Health Information Library, LINK-up to find out how.
Do you have a complaint and want to file a grievance? The KFHC member app is your LINK to letting us know your complaint.
