About MyCHOIS

The My Collaborative Health Outcomes Information System (MyCHOIS) is a tool designed to help you manage and track your health information and view other recovery-oriented tools. In the MyCHOIS Consumer app, you can access:
• Crisis Hotlines: Find contact information for crisis hotlines that you can either call, text, or chat for support if you are in a suicidal crisis or emotional distress.
• Crisis Resources: View links to crisis services and educational websites, along with contact information for statewide warmlines and information lines.
• Recovery Library: View educational materials and recovery tools developed by psychologist and disability-rights advocate Pat Deegan geared to support, whole-health, self-advocacy, and medication.
• My Treatment Data: Access your personal health record! Your treatment data in MyCHOIS comes from the New York State (NYS) Office of Mental Health Psychiatric Services and Clinical Knowledge Enhancement System (PSYCKES) database. Any of your health services paid for by Medicaid in the past 5 years will be included in your record. Your online record also includes your health information from other NYS databases, and new databases may be added. View data on your health history including medications, outpatient services, hospital visits, diagnoses, and more.
• Documents: Create a safety plan or view other completed documentation to help guide informed decisions about your care.
