anticoagulants.

1. Warfarin dosing Calculator
This application functions is a tool for Warfarin dosage
adjustment, to help make adjustment to therapy for patients
currently receiving warfarin who are no longer in the therapeutic
range at admission or during clinic visit.
2. DOAC dosing recommendation
This application functions is a tool for DOAC dosage
recommendation in 2 situations. (Atrial fibrillation and Venous
thromboembolism)
3. Perioperative management
This application functions is guide for perioperative
management to
- Assist physicians is the simultaneous of procedure-
related bleeding risk and underlying risk of thrombosis.
- Guide decisions regarding the interruption of
anticoagulant and the use of anticoagulant bridging and
when to restart anticoagulants.
- Provide details guidance for drug dosing and laboratory
monitoring in the peri-procedural period.

decision about bleeding management for patient receiving
