OCS Q allows you to easily create your own forms for capturing data.
You need to be an existing OCS customer and enter a username and password to use the app.

Create dynamic forms that adapt to your answers for many types of audits, including quality assurance checks, site inspections, health and safety checklists etc.
The simple web based form designer allows you to drag and drop elements on to the form.
Add elements to capture:
- single line text
- multi line text
- images
- bar codes
- signatures
- dates
- selection buttons (with any text and color)
- numeric values
- drop downs
- lookup elements
- time stamp buttons
and many more
Create nested elements that are only visible if the parent element has a specific value.
Add validation to forms like min and max values and whether it should be compulsory or not.
Don’t like the placement? Just drag and drop it.
Added the wrong type of element? Just change its type. No need to worry about recreating nested elements again.
Want to dynamically add multiple values of an element or a group of elements (an asset, juice machine, vehicle etc. with multiple fields)? No problem. Just set the repeat count min and max values.
Add as many pages as you want to your audit. Easily jump to a page for editing, or rearrange pages.
Use a similar element or group of elements over and over? Just drag and drop it (with all its child elements) onto the component palette for later use or to copy it to another page.
Audits will calculate scores and email you a PDF on completion by default. For more complex business requirements, form submission can integrate to back end systems allowing everything from task automation, job card creation, issuing tasks, closing tasks to customized reports to be created.
Forms can be set to work in offline or online mode. This allows a workforce to capture data off site and sync to the server using the office WiFi.
All submitted data is stored online so you always have access to it.
Forms can be categorized into groups to allow only users linked to those groups to be able to complete them.
Users can be linked to multiple roles, giving them access to different menus, forms or groups of forms.
Only users that have been given permission can add or change form templates, giving only the right people access.
Forms can be developed that can read lookup data from a back end system, filtered by the current user based on assigned permissions or linked data (user linked to regions, buildings, contracts, departments or whatever). You can even have lookups filtered by other lookups, like Locations filtered by Buildings.
Custom forms can include more in depth logic and validation that can be developed for you specific needs.
