The TELUS Collaborative Health Record Intake (TELUS CHR Intake) is the starting point for your patient to be an active member of their health journey.

With the TELUS CHR Intake, you can:
- Register new patients
- Set up questionnaires for patients
- View questionnaire responses directly in the patient file in the CHR
- Notify clinicians when questionnaires are complete
