⚠️ IMPORTANT: This app is exclusively for patients enrolled in a CareTrack Remote Patient Monitoring (RPM) or Chronic Care Management (CCM) program through a participating healthcare provider.

This app requires CareTrack-provided medical devices to function. It does not work as a standalone app and is not compatible with personal or third-party medical devices. If you are not enrolled in a CareTrack program, this app will not work for you.

CareTrack Connect: Simple Daily Health Monitoring from Home

CareTrack Connect makes it easy to stay on top of your health between visits. The app syncs with medical devices provided by CareTrack as part of your enrolled care program to track blood pressure, blood glucose, weight, oxygen saturation, and other important vitals.

How It Works:
1. Your healthcare provider enrolls you in a CareTrack RPM or CCM program
2. CareTrack provides you with compatible medical monitoring devices
3. Download the app and sign in with the credentials provided to you
4. Your devices automatically sync readings to the app and your care team

Who Can Use CareTrack Connect?

This app is ONLY available to patients who:
• Are currently enrolled in a Remote Patient Monitoring or Chronic Care Management program
• Have received CareTrack-provided medical devices from their healthcare provider
• Have been given login credentials by their care team

If you are not enrolled in a CareTrack program, please contact your healthcare provider to learn if CareTrack services are available to you.

• CareTrack-provided medical monitoring devices (provided by your healthcare provider)
• Login credentials from your care team

This app cannot function without CareTrack-provided devices and an active enrollment.
